Job summary

RHA Health Services, LLC in Fayetteville, NC is seeking a START Coordinator to support individuals with developmental disabilities and mental health challenges through coordinated community-based services.

The role focuses on crisis planning, case management, and collaboration with families and care teams to ensure integrated, person-centered care. A Bachelor's in Social Work or related field is required, with a Master’s preferred; travel is required.

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required.
• Master’s degree a plus.
• Minimum 2 years experience working with persons with intellectual and/or developmental disabilities required.
• 1-year experience if paired with Master’s degree.
• Must meet requirements for both Qualified Developmental Disability Professional and Qualified Mental Health Professional.
• Must have direct support experience supporting individuals with mental health and developmental disability.
• Must have experience with crisis management and crisis counseling.
• Valid driver’s license CPR, first aid, NCI certification and others that may be specified and/or required by future company regulations.
• Must have proficiency in company sponsored training and certifications and privileging if required.
• Ability to Travel #RHAIDD

Responsibilities

• Develops and revises the cross systems crisis plans in a timely fashion.
• Monitors the implementation of the plan.
• Performs Case Management functions of linking and arranging for services and referrals.
• Provides families and stake holders with individual‑specific training and training in the knowledge, skills, and abilities required by the population and age to be served.
• Provides consultation to the system on the clinical needs of the individual's care.

Education
