Job summary

RHA Health Services, LLC is hiring for a START Coordinator to support individuals with developmental disabilities and mental health challenges in Craven and Onslow counties, with travel to surrounding areas. Salary is $56,000.

The role focuses on crisis planning, case management, and family training within an integrated care team to deliver community-based intervention and prevention services.

Qualifications

• Must have direct support experience supporting individuals with mental health and developmental disability diagnosis.
• Must have experience with crisis management and crisis counseling.
• Must meet requirements for both Qualified Developmental Disability Professional and Qualified Mental Health Professional.

Responsibilities

• Develops and revises the cross systems crisis plans in a timely fashion.
• Monitors the implementation of the plan.
• Performs Case Management functions of linking and arranging for services and referrals.
• Provides families and stake holders with individual-specific training and training in the knowledge, skills, and abilities required by the population and age to be served.
• Provides consultation to the system on the clinical needs of the individual's care.
