Job summary

RHA Health Services, LLC is seeking a Therapeutic In-Home Coach to support participants in the START Program in Fayetteville, NC and surrounding areas. The role provides community-based intervention, crisis support, and collaboration with families and care teams.

The ideal candidate holds a Bachelor's degree in psychology, social work, or a related human service field and has experience with intellectual/developmental disabilities or mental health populations.

Qualifications

• Bachelor’s degree in psychology, social work or other human service related field.
• 2 years’ experience with population served preferred.
• Ability to work in home/community settings.
• Strong communication and crisis intervention skills.

Responsibilities

• Coaches work within the home or other community settings to create and implement person specific and crisis therapeutic interventions and communicate these interventions with primary caregivers.
• Coaches contribute to the overall crisis stabilization and prevention of the individual and family by providing hands-on assistance and education to families.
• Coach participates as part of comprehensive START services that collaborate with individual's primary provider and clinical home, community crisis services, family and others.
• Offer consultation and hands-on education with entire family, support system to create a safe, nurturing, and meaningful environment for families and individuals with development disabilities.
• Assists individuals with implementation of daily activities, behavioral health needs, and health, wellness, and leisure activities.
• Monitors individuals for any mental health or behavioral symptoms and provides crisis prevention, intervention and de-escalation support in accordance with crisis guidelines and person's plan.
• Maintains timely documentation of session and visits notes and other data.
• Coaches work with families and individuals to work on identified needs/skills to assist with strategies to improve coping mechanisms.
