Job summary

Easterseals Southern California is seeking a START Coordinator to provide comprehensive case management, crisis support, and coordination of care using the START service model. You will support individuals at risk, advocate for services, and collaborate with leadership to ensure high-quality care.

The role requires a Master’s degree in a relevant field, CPR certification, and 2 years of experience with developmental disabilities and behavioral health needs.

Qualifications

• Master’s degree in social work, psychology, counseling or human services.
• Current CPR certification is required.
• Two years of professional experience with developmental disabilities and behavioral health needs.

Responsibilities

• Provide case management, crisis support, coordination of care using START model.
• Train and supervise Therapeutic Coaches and providers.
• Lead START team meetings and review potential crises.
• Coordinate crisis plans and assessment outcomes with START team.
• Maintain active caseload and participate in START Coordinator certification.
• Provide on-call support and participate in the National START network.
