Job summary

YAI seeks a dedicated professional to manage a START caseload of 20–25 individuals, conducting home visits and telehealth assessments as needed. The role includes coordinating crisis plans, documenting outcomes, and collaborating with the clinical leadership to ensure compliant service delivery.

The position requires a Master’s degree in Mental Health or related field, plus 2+ years of relevant experience and START Coordinator Certification within 12 months.

Qualifications

  • Master’s degree in Mental Health, Psychology, Counseling, Social Work or other Human Service field.
  • At least two (2) years of experience with the I/DD population and/or behavioral health needs.
  • Successful completion of START Coordinator Certification training within 12 months of appointment.

Responsibilities

  • Maintains an active caseload of up to 20–25 individuals referred for START Services and provides systemic consultation.
  • Conducts regular outreach visits to individuals' home, day, or vocational settings via in-person or telehealth visits.
  • Coordinates support meetings and crisis plan development for START, including communication with Medical/Clinical Directors.
  • Completes and maintains documentation and enters required data into START information systems on a timely basis.
  • Participates in START leadership and clinical team meetings and provides on-call support as scheduled.
