Job summary

JEVS Human Services in Philadelphia seeks a START Coordinator to lead community-based assessment, crisis prevention, planning, and therapeutic coordination, stabilizing individuals in crisis and strengthening natural support systems. Qualifications

• Master’s degree in a human services field and eligibility for certification.
• Minimum of 3 years supporting individuals with IDD and co-occurring mental health needs.
• Strong clinical judgment, organizational skills, and collaborative communication.
• Must obtain START Coordinator certification within 12 months.

Responsibilities

• Manage a caseload of individuals with dual diagnoses and develop CSCPIPs.
• Conduct assessments and coordinate therapeutic supports with the clinical team.
• Coordinate crisis prevention, planning, and on-call crisis response.
• Serve as a clinical liaison across behavioral health, developmental services, and other systems.
• Ensure accurate documentation in SIRS and EMR; track outcomes.
