Job summary

RHA Health Services, LLC is hiring a START Coordinator to support individuals with developmental disabilities and mental health challenges in a community-based program. The role focuses on crisis plan development, case management, and training for families and stakeholders.

The position requires a Bachelor's in a relevant field, 2 years of experience with the target population, and travel as needed. A Master’s degree is a plus and CPR/first aid certifications may be required.

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required; Master’s degree a plus.
• Minimum 2 years experience with persons with intellectual/developmental disabilities; 1 year if paired with Master’s.
• Must meet requirements for QDDP and QMHP; must have crisis management experience.
• Valid driver’s license required.

Responsibilities

• Develops and revises cross systems crisis plans in a timely fashion.
• Monitors the implementation of the plan.
• Performs Case Management functions: linking and arranging services/referrals.
• Provides families with individual-specific training and knowledge for the population served.
• Provides clinical consultation on individuals’ care.
