Job summary

The Sage Creek Post-Acute in Las Vegas is seeking a Staff Physical Therapy Assistant to support the Physical Therapy team in delivering patient-centered care according to the plan of care, helping patients regain mobility and independence.

You will document treatment notes daily, assist with therapeutic activities, communicate with the supervisor and health team, train Restorative Aides, and help maintain a clean, organized treatment area, while ensuring safety and compliance with state

Qualifications

• Licensed as a Staff Physical Therapy Assistant in the state.
• Proficient in computer skills and electronic health records.
• Ability to read and follow treatment plans and policies.

Responsibilities

• Treat patients as directed by Physical Therapist.
• Record daily treatment notes and weekly progress notes.
• Assist with cleaning and maintenance of treatment area.
• Communicate progress with supervisor and health team.
• Instruct families and nursing staff in maintenance and discharge prep.
• Participate in in-services and training programs.
