Job summary

Lindsay Gardens Nursing and Rehabilitation is seeking a Staff Physical Therapist to evaluate and treat residents, develop individualized plans, and collaborate with families, physicians, and the health care team.

You will supervise PT assistants and complete compliant documentation while participating in quality improvement initiatives and staff training. Licensure in the state is required with ongoing CE requirements, focusing on safe mobility and fall prevention.

Qualifications

• Licensed Physical Therapist in the state with continuing education requirements.
• Ability to read and comprehend policy and procedure manuals.
• Proficient in computer skills for documentation and data entry.
• Ability to supervise PT assistants, aides, and students.
• Strong communication with residents, families, and healthcare team.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and support staff in resident care activities.
• Maintain timely, accurate documentation in accordance with policies and payer requirements.
• Educate residents and families on home exercise programs and safety.
• Participate in care conferences and quality improvement initiatives.
