Job summary

Polaris Pharmacy Services is hiring a Staff Pharmacist to oversee day-to-day dispensing duties for a long-term/post-acute care setting. The role emphasizes adherence to quality standards and compliance with state/federal guidelines.

Qualifications

• Current Pharmacist license active and in good standing in the state of practice.
• Management experience preferred
• Bachelor's degree or Pharmacy Doctorate from an accredited School of Pharmacy
• Minimum of two (2) year's recent experience in an acute-care setting or one (1) year experience as a production pharmacist preferred (not required).
• Experience and knowledge base of intravenous admixtures services and intravenous therapy desired.

Responsibilities

• Verify completeness and accuracy of all new and refill labels and orders.
• Maintain resident database with current medical history and medications.
• Assist in ordering, handling, receiving, documenting, and storage of controlled substances; maintain inventory for schedule II drugs.
• Reconcile Controlled Substances inventory per regulations.
• Provide clinical expertise by responding to drug-related questions from staff and customers.
• Document communications with physicians, nurses, and patients in medical records.
• Prepare and supervise sterile IV drug products or solutions.
• Provide evening, weekend, and holiday coverage as required.
• Follow HIPAA and government regulations.
• Other duties as assigned; adapt to business needs.
