Job summary

Polaris Pharmacy Services of Ohio is hiring a Staff Pharmacist to handle daily dispensing duties for a long-term/post-acute care pharmacy. The role requires adherence to quality standards and regulatory guidelines, with a schedule of 2pm - 10:30pm and every third weekend.

You will verify labels, maintain resident medical histories, manage controlled substances inventory, respond to drug-related questions, and oversee IV drug preparation. Strong communication and detail orientation are essential.

Qualifications

• Active pharmacist license in state of practice.
• Experience in acute-care settings preferred.
• Ability to handle evenings, weekends, holidays as needed.
• Knowledge of intravenous admixtures and IV therapy is desired.

Responsibilities

• Verify accuracy of new and refill labels and orders.
• Maintain resident medical histories and related data.
• Manage controlled substances inventory and scheduling.
• Reconcile inventory per state and federal regulations.
• Provide clinical responses to drug-related questions.
• Document communications in the patient medical record.
• Prepare and supervise sterile IV products.
• Provide coverage during evenings, weekends, and holidays.
