Job summary

Sherwood Oaks Post Acute is seeking a Staff Occupational Therapist Assistant to support the Occupational Therapist in delivering resident-centered care. The role includes direct patient care, documentation, and collaboration with the multidisciplinary team to ensure quality outcomes.

The OTA will participate in care conferences, assist with training for Restorative Aides, and educate residents and families on treatment goals and home exercise programs.

Qualifications

• Associate Degree from an accredited COTA program.
• Proficiency with electronic health record systems including Net Health and Point Click Care.
• Ability to read and interpret procedures and manuals.
• Excellent communication with patients, families, and care teams.

Responsibilities

• Assist the Staff Occupational Therapist with resident care and treatment.
• Document accurately and on time in accordance with facility and payer requirements.
• Maintain a safe, clean, and infection-controlled treatment environment.
• Participate in care conferences and rehabilitation meetings as needed.
• Assist with training Restorative Aides and educate residents and families on goals and home programs.
• Communicate progress and discharge planning updates to the care team and physicians.
• Maintain confidentiality of resident and company information.
• Recommend adaptive equipment to promote independence in ADLs.
