Job summary

Alston Wilkes Society, Inc. is seeking a Case Manager to support residents at AWVH. Under the direction of the Director of VA Facilities and Programs, you will provide outreach, individual case management, and coordinate supportive services to move veterans toward stable housing.

You will collect and report data per NOFA requirements and strive for rapid placement within 90 days, leveraging community resources and VA support services to assist low-income veterans and homeless individuals.

Qualifications

• Bachelor’s degree in a social science or related field; graduate degree or licensure in Social Work preferred.
• Three years of experience with low-income populations, veterans or homeless populations preferred.
• Writing sample demonstrating clinical documentation will be required.

Responsibilities

• Provide outreach and case management to AWVH residents.
• Facilitate rapid transition to permanent housing within 90 days under the grant.
• Collect data and report in HMIS to meet NOFA requirements.

Job description

Under the direction and supervision of the Director of VA Facilities and Programs, to provide outreach and case management services to the residents of the AWVH to facilitate rapid transition (within 90 days) to permanent housing under a VA Supportive Services for Veteran Families grant.
