Job summary

Mount Rogers Community Services seeks a Sponsored Residential/Services Facilitator Supervisor to oversee day-to-day operations of sponsored homes and coordinate care planning for individuals served.

You will ensure compliance with VA Licensure, Medicaid, and Human Rights standards, train EORs, monitor service delivery, and support coordinators in implementing person-centered plans within a team-based environment.

Qualifications

• Must meet one of the criteria: High School diploma + 2 years direct care; or Associate’s degree + 2 years direct care.
• Experience with individuals with intellectual/developmental disabilities or co-occurring conditions preferred.

Responsibilities

• Monitor adherence to VA Licensure, Medicaid and Human Rights Standards.
• Review and summarize needs for Monthly summaries and Individual Service Plans.
• Monitor service delivery for quality and regulatory compliance.
• Coordinate person-centered practices with support coordinators and providers.
• Promote use of natural supports in planning services and supports.
• Assist in 24-hour on-call coverage for emergencies and provider consultations.
• Ensure accurate billing is submitted electronically each month.
