Job summary

Community Healthcare of Texas is seeking a compassionate Social Worker in Dallas to provide therapeutic support, education, and resource information to patients and families facing serious illness within hospice care.

You will advocate for patients' needs, contribute to the care plan, document visits, and connect families with community resources while collaborating with the care team in a respectful, patient-centered environment.

Qualifications

• Master's degree in Social Work is required.
• Reliable transportation and a valid driver's license are required.
• Hospice or healthcare experience is preferred.
• Proficient computer skills are required.

Responsibilities

• Assess psychosocial needs of the patient and family.
• Provide therapeutic counseling and education on end-of-life issues.
• Contribute to and execute the Plan of Care with the family/patient.
• Document visits and contacts daily in accordance with Policy.
• Provide referrals to community resources.
• Performs other duties as assigned.

Community Healthcare of Texas is currently recruiting for a Social Worker.

POSITION SUMMARY

To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers.

ESSENTIAL FUNCTIONS

• Assess the psychosocial needs of the patient and family members
• Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues
• Contribute to and execute the Plan of Care with the family/patient
• Record daily documentation of visits and contact in accordance with Policy
• Provides referrals to community resources
• Performs other duties as assigned

POSITION QUALIFICATIONS

• Master's degree in Social Work required
• Reliable transportation and a valid driver's license.
• Hospice or healthcare experience preferred.
• Proficient skills using a computer are required
