Community Healthcare of Texas is seeking a Social Worker to provide therapeutic support, education, and resource information to patients and families coping with serious illness. You will advocate for patients' wishes within the care team and coordinate with families and caregivers to align care goals.

Responsibilities include assessing psychosocial needs, delivering counseling on end-of-life issues, documenting visits, and linking families with community resources, while ensuring compassionate

Qualifications

• Master's degree in Social Work required.
• Reliable transportation and a valid driver's license.
• Hospice or healthcare experience preferred.
• Proficient skills using a computer are required.

Community Healthcare of Texas is currently recruiting for a Social Worker.

POSITION SUMMARY

To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers.

ESSENTIAL FUNCTIONS

• Assess the psychosocial needs of the patient and family members
• Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues

• Record daily documentation of visits and contact in accordance with Policy
