Job summary

Community Healthcare of Texas is seeking a Social Worker to provide therapeutic support and education to patients with serious illnesses and their families in a hospice setting.

You will advocate for patients’ needs, participate in the Plan of Care with the care team, and document visits in accordance with policy. A Master’s degree in Social Work and computer proficiency are required; hospice experience preferred.

Qualifications

• MSW degree required.
• Reliable transportation and a valid driver’s license.
• Hospice or healthcare experience preferred.
• Proficient skills using a computer are required.

Responsibilities

• Assess psychosocial needs of patient and family.
• Provide therapeutic counseling and emotional support; education on end-of-life issues.
• Contribute to and execute the Plan of Care with family/patient.
• Record daily documentation of visits per policy.

POSITION SUMMARY

To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers.

ESSENTIAL FUNCTIONS

• Assess the psychosocial needs of the patient and family members
• Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues
• Contribute to and execute the Plan of Care with the family/patient
• Record daily documentation of visits and contact in accordance with Policy
• Provides referrals to community resources
• Performs other duties as assigned
