Qualifikationen

• Current state licensure in the state practicing.
• Experience in long term care or skilled nursing, preferred.
• Possess effective communication skills to maintain positive relationships with residents, families, staff, physicians, consultants, providers, and the community.
• Current knowledge of local, state and federal guidelines and regulations.

Aufgaben

• Direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to guidelines.
• Provides professional treatment while using therapeutic techniques for rehabilitation of speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plans activities and provides therapeutic services for speech and language disorders.
• Serves as patient and rehabilitation advocate; participates in quality assurance and customer service activities.
• Perform patient evaluations and develop treatment plans, including short-term and long-term goals, aligned with physician orders and patient condition.
• Carry out patient care plans or portions thereof, determining progression within established care plans.
• Other duties, responsibilities and activities may change or be assigned.
