Job summary

Apploi in Kentucky seeks a Speech Language Pathologist to provide in-house therapy at a Skilled Nursing Facility. The role includes planning, implementing, and supervising SLP services in line with CMS and facility standards.

The successful candidate will hold a Master’s in Speech Language Pathology, be CCC-SLP certified, and hold an active license. Excellent communication with residents, families, and staff is essential for high-quality care.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency for SLP.
• Active SLP license in the state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop a treatment plan with target list, short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screening protocols to identify needs.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice

Ongoing continuing education and professional development to maintain licensure and certification
