• Experience in long-term care (Skilled Nursing Facility) preferred.

Responsibilities

• Collaborate with therapy teams and Rehab Department.
• Complete evaluations and all necessary documentation.
• Provide speech therapy per treatment plans and prescriptions.
• Address speech, language, and dysphagia needs to improve communication abilities.
• Manage workflow including documentation and notes in the electronic records system.
• Align therapy plans with Medicaid/Medicare billing considerations.
