It offers 24-hour Medicare-certified care in an 83-bed facility near downtown Livermore. The Speech Therapist provides and directs all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to organizations guidelines, professional standards and community needs. Serves as a patient and rehabilitation advocate for current prospective patients, to all internal and external customers. Participates in quality assurance and customer service activities.

• Conduct comprehensive assessments to evaluate patients' speech and language abilities.
• Develop and implement individualized treatment plans, targeting specific speech, language, and swallowing goals.
• Provide therapeutic interventions, including but not limited to, articulation therapy, language development therapy, voice therapy, and cognitive training.
• Monitor and document patients' progress and make adjustments to treatment plans, as necessary.
• Collaborate with other healthcare professionals, such as occupational therapists, physical therapists, and nurses, to coordinate patient care.
• Educate patients, families, and caregivers about speech and language disorders, treatment strategies, and home exercises.
• Participate in team meetings and case conferences to discuss patients' progress and make recommendations for discharge planning.

QUALIFICATIONS/SPECIFIC REQUIREMENTS:

• Graduate of an approved curriculum in Speech and Language Pathology.
• Is in good standing with all regulatory agencies and licensing boards.
• Current, active license as Speech Pathologist, and is able to practice unencumbered.
• CPR/BLS Active Certification
• Skilled Nursing Facility strongly preferred
