Job summary

Redwoods Post Acute in Willow Glen, San Jose, CA, seeks a dedicated Speech Language Pathologist to join our 152-bed skilled nursing facility team serving geriatric patients. You will assess, diagnose, and treat speech, language, and swallowing disorders.

Responsibilities include individualized treatment plans, progress tracking, team collaboration with therapists and nurses, and family education. Must hold CCC-SLP, state licensure, CPR, and prefer SNF experience; we offer comprehensive benefits

Qualifications

• Valid state licensure as a Speech Language Pathologist.
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP).
• Active CPR Certification.
• Interest in geriatric populations and long-term care.
• New graduates with CF supervision welcome.

Responsibilities

• Conduct comprehensive assessments of patients' speech and language abilities.
• Develop and implement individualized treatment plans.
• Provide articulation, language, voice, and cognitive therapies.
• Monitor progress and adjust treatments as needed.
• Collaborate with therapists and nurses for coordinated care.
• Educate patients, families, and caregivers about disorders and at-home exercises.

RESPONSIBILITIES:

• Conduct comprehensive assessments to evaluate patients' speech and language abilities.
• Develop and implement individualized treatment plans, targeting specific speech, language, and swallowing goals.
• Provide therapeutic interventions, including but not limited to articulation therapy, language development therapy, voice therapy, and cognitive training.
• Monitor and document patients' progress and make adjustments to treatment plans as necessary.
• Collaborate with other healthcare professionals, such as occupational therapists, physical therapists, and nurses, to coordinate patient care.
• Educate patients, families, and caregivers about speech and language disorders, treatment strategies, and home exercises.
• Participate in team meetings and case conferences to discuss patients' progress and make recommendations for discharge planning.

QUALIFICATIONS:

• New passer/graduates and CF supervision are welcome
• Previous experience as Sa peech Language Pathologist (SLP) or Speech Therapist (ST) is a plus
• With Skilled Nursing or Long-Term Care experience is a plus
• Valid state licensure as a Speech Language Pathologist
• Active CPR Certification
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)
• Is in good standing with all regulatory agencies and licensing boards is required
• Interest in working with the geriatric population
