Job summary

The Vineyards Healthcare Center in Livermore, California, is seeking a Speech Therapist to provide comprehensive evaluation, treatment, and documentation for residents of an 83-bed skilled nursing facility.

The role collaborates with Occupational Therapists, Physical Therapists, and nurses, and includes educating families and participating in discharge planning to ensure the best patient outcomes.

Qualifications

• Graduate of an approved curriculum in Speech and Language Pathology.
• Active licensure as a Speech-Language Pathologist with no encumbrances.
• CPR/BLS Active Certification.
• Experience in a Skilled Nursing Facility is strongly preferred.

Responsibilities

• Conduct comprehensive assessments of speech and language abilities.
• Develop and implement individualized treatment plans and goals.
• Provide articulation, language, voice, and cognitive therapy.
• Monitor progress and adjust treatment plans accordingly.
• Collaborate with occupational and physical therapists and nurses.
• Educate patients, families, and caregivers on strategies and home exercises.
• Participate in team meetings and discharge planning discussions.
