Job summary

The Vineyards Healthcare Center in Livermore, California, is seeking a full-time Occupational Therapist to evaluate and treat patients across acute care, skilled nursing, short-term rehab, and outpatient settings. You will collaborate with a multi-disciplinary team and advocate for patients and families.

Responsibilities include treatment planning, documentation, and quality improvement activities, reporting to the Director of Rehabilitation Services.

Qualifications

• Graduate of an American Occupational Therapy Association (AOTA)–accredited OT program.
• California license to practice Occupational Therapy is required.
• Experience in skilled nursing or rehabilitation settings preferred.
• Experience in acute care, skilled nursing, or outpatient settings preferred.

Responsibilities

• Evaluate and treat patients in acute care, skilled nursing, short-term rehabilitation, and outpatient settings.
• Provide and direct all aspects of quality patient care, including evaluation, treatment planning, goal setting, family education, and documentation.
• Serve as a patient and rehabilitation advocate for current and prospective patients and for internal and external customers.
• Oversee services provided by OT assistants and aides; participate in quality assurance activities; report to the Director of Rehabilitation Services.

Job description

The Vineyards Healthcare Center is a skilled nursing facility located at 76 Fenton Street, Livermore, California. It offers 24-hour Medicare-certified care in an 83-bed facility near downtown Livermore. It has received high-performing ratings in both short-term rehabilitation and long-term care from the U.S. News & World Report. The center is committed to providing individualized, holistic care through warm and meaningful interactions, aiming to help residents achieve their best outcomes and ensure their comfort is a top priority.

The Occupational Therapist will work Full Time, Days, and will evaluate and treat patients in acute care, skilled nursing, short-term rehabilitation, and outpatient settings. Provides and directs all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education, and documentation according to organizations guidelines, professional standards, and community needs. Serves as a patient and rehabilitation advocate for current and prospective patients, and to all internal and external customers. Actively participates in quality assurance and customer service activities for the rehabilitation department.

Assist in quality improvement and new programming initiative development.

As an Occupational Therapist will work as part of a multi-disciplinary care team.

Oversees the services provided by Certified Occupational Therapy Assistants and aides.

Report to the Director of Rehabilitation Services.

QUALIFICATIONS/SPECIFIC REQUIREMENTS:

EDUCATION: Graduate of a program accredited by the American Occupational Therapy Association (AOTA)

EXPERIENCE:

With Occupational Therapy, Experience is PREFERRED

With Skilled Nursing, Experience is PREFERRED

LICENSES/CERTIFICATIONS: California license to practice Occupational Therapy REQUIRED
