Qualifications

• Graduate of an accredited Speech-Language Pathology program.
• Current Massachusetts Speech-Language Pathologist license.
• Two years of experience in acute care, rehab or homecare as a Speech Therapist.
• Comfort with various computer systems and handheld clinical devices.
• Ability to travel within the Mass General Brigham Home Care geographic area.

Responsibilities

• Evaluate patients and develop individualized treatment plans for home-based care.
• Provide skilled speech-language and swallowing therapy in patients’ homes.
• Educate patients, families, and caregivers on treatment strategies and recommendations.
• Coordinate care with physicians, nurses, and the interdisciplinary team to ensure smooth transitions to outpatient services.
• Document evaluations, progress, and care coordination activities and monitor patient progress.
