Job summary

Mass General Brigham Home Care, Inc. is seeking a Speech-Language Pathologist to provide in-home evaluations and therapy across diverse patient populations.

You will design individualized treatment plans, perform comprehensive assessments, and collaborate with physicians, nurses, and caregivers to ensure continuity of care. You’ll work within a supportive team, manage your own schedule, and travel to patients’ homes across the assigned Massachusetts service area.

Qualifications

• Licensed Speech-Language Pathologist in MA.
• Two years’ experience in acute care, rehab or homecare as an SLP.
• Graduate from an accredited SLP program.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled speech-language and swallowing therapy in the home.
• Educate patients, families, and caregivers on treatment strategies.
• Assess communication, cognition, and swallowing needs and adjust plans as needed.

Most of your visits will be in Hathorne, Lynnefield, Middleton, Peabody, Andover, Topsfield, Boxford, Hamilton, Wentham, Bedford, Billerica, Burlington, Concord, Lawrence, Lexington, Lincoln, Reading, Stoneham, Tewksbury, Wakefield, Wilmington, Winchester, Woburn.

• A schedule that works for you: Part-time (32-hours), 4-5 visits per day, Monday–Thursday or Tuesday-Friday with no evenings, weekends or holidays with the autonomy to manage your own schedule and appointments while providing care to patients in their homes.

What you’ll do

As a Speech-Language Pathologist, you’ll evaluate patients, develop individualized treatment plans, provide skilled therapy, and work closely with patients, families, caregivers, and an interdisciplinary team.

• Complete comprehensive speech, language, cognitive, and swallowing evaluations
• Develop and implement individualized treatment plans based on patient needs and goals
• Provide skilled speech-language and swallowing therapy in the home
• Educate patients, families, and caregivers on treatment strategies and recommendations
• Assess and address communication, cognition, and swallowing needs
• Make recommendations for appropriate compensatory strategies, adaptive techniques, and equipment as indicated
• Coordinate care with patients, families, physicians, nurses, and other members of the interdisciplinary team to initiate and support transition to outpatient services as appropriate
• Document evaluations, treatment, progress, and care coordination activitiesMonitor patient progress and modify treatment plans as appropriate
• Collaborate with the interdisciplinary team to support safe, effective, patient-centered care
• Deliver high-quality care that helps patients maximize their independence and quality of life

What you’ll bring

• Graduate of an accredited Speech-Language Pathology program
• Current Massachusetts Speech-Language Pathologist license
• Two year’s experience in acute care, rehab or homecare as a Speech Therapist
• Comfort working with various computer systems; experience with handheld clinical devices preferred
• Strong interpersonal and customer service skills with the ability to work with diverse populations
• Strong organization and prioritization skills with the ability to adapt in a dynamic environment
• Ability to work independently and remain self-directed in unpredictable situations
• Ability to collaborate effectively as part of an interdisciplinary care team
• Willingness to travel within the MGB Healthcare at Home geographic area
• Excellent communication skills—written, verbal, and electronic
