Job summary

Mass General Brigham Home Care, Inc. is seeking a Speech-Language Pathologist to deliver home-based assessments and therapy across a broad patient population. You will develop individualized treatment plans and work within an interdisciplinary team to maximize patient independence.

The role offers a hybrid work arrangement, a 32-hour schedule, and opportunities for ongoing training and professional development within the Mass General Brigham system.

Qualifications

• Graduate of an accredited Speech-Language Pathology program.
• Current Massachusetts Speech-Language Pathologist license.
• Two years' experience in acute care, rehab, or homecare as a Speech Therapist.
• Comfort with multiple computer systems and handheld devices is preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans.

Most of your visits will be in Hathorne, Lynnefield, Middleton, Peabody, Andover, Topsfield, Boxford, Hamilton, Wentham, Bedford, Billerica, Burlington, Concord, Lawrence, Lexington, Lincoln, Reading, Stoneham, Tewksbury, Wakefield, Wilmington, Winchester, Woburn.

What you’ll do

As a Speech-Language Pathologist, you'll evaluate patients, develop individualized treatment plans, provide skilled therapy, and work closely with patients, families, caregivers, and an interdisciplinary team.

• Complete comprehensive speech, language, cognitive, and swallowing evaluations
• Develop and implement individualized treatment plans based on patient needs and goals
• Provide skilled speech-language and swallowing therapy in the home
• Educate patients, families, and caregivers on treatment strategies and recommendations
• Assess and address communication, cognition, and swallowing needs
• Make recommendations for appropriate compensatory strategies, adaptive techniques, and equipment as indicated
• Coordinate care with patients, families, physicians, nurses, and other members of the interdisciplinary team to initiate and support transition to outpatient services as appropriate
• Document evaluations, treatment, progress, and care coordination activities
• Monitor patient progress and modify treatment plans as appropriate
• Collaborate with the interdisciplinary team to support safe, effective, patient-centered care
• Deliver high-quality care that helps patients maximize their independence and quality of life

Qualifications

What you’ll bring

• Graduate of an accredited Speech-Language Pathology program
• Current Massachusetts Speech-Language Pathologist license
• Two year's experience in acute care, rehab or homecare as a Speech Therapist
• Comfort working with various computer systems; experience with handheld clinical devices preferred
• Strong interpersonal and customer service skills with the ability to work with diverse populations
• Strong organization and prioritization skills with the ability to adapt in a dynamic environment
• Ability to work independently and remain self-directed in unpredictable situations
• Ability to collaborate effectively as part of an interdisciplinary care team
• Willingness to travel within the MGB Healthcare at Home geographic area
• Excellent communication skills-written, verbal, and electronic
