Job summary

Mass General Brigham Home Care is seeking a Speech-Language Pathologist to provide in-home evaluations and therapy, delivering services to a diverse patient population across multiple communities in Massachusetts.

You'll work independently with support from a strong interdisciplinary team, managing your own schedule for part-time visits (32 hours per week) with no evenings, weekends, or holidays, and ongoing opportunities for professional development.

Qualifications

• Graduate of an accredited Speech-Language Pathology program.
• Current Massachusetts Speech-Language Pathologist license.
• Two year's experience in acute care, rehab or homecare as a Speech Therapist.
• Comfort working with various computer systems; experience with handheld clinical devices preferred.
• Strong interpersonal and customer service skills with the ability to work with diverse populations.
• Strong organization and prioritization skills with the ability to adapt in a dynamic environment.

• Complete comprehensive speech, language, cognitive, and swallowing evaluations
• Develop and implement individualized treatment plans based on patient needs and goals
• Provide skilled speech-language and swallowing therapy in the home
• Educate patients, families, and caregivers on treatment strategies and recommendations
• Assess and address communication, cognition, and swallowing needs
• Make recommendations for compensatory strategies, adaptive techniques, and equipment as indicated
• Coordinate care with patients, families, physicians, nurses, and other members of the interdisciplinary team to initiate and support transition to outpatient services as appropriate
• Document evaluations, treatment, progress, and care coordination activities
• Monitor patient progress and modify treatment plans as appropriate
• Collaborate with the interdisciplinary team to support safe, effective, patient-centered care
• Deliver high-quality care that helps patients maximize their independence and quality of life
