Job summary

Home & Health Care Management in Northern California seeks a licensed Speech-Language Pathologist to deliver in-home evaluations, create and implement treatment plans, and provide therapy for communication, cognition, voice and swallowing disorders.

The role is part-time with potential for expanded hours and requires 100% travel to clients’ homes across counties such as Butte, Shasta, Yuba, Colusa, Glenn and Tehama. California SLP license, CPR, and a valid driver’s license are essential.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Active California SLP license.
• Current CPR certification.
• Minimum of 1 year experience, preferably in home health or medical setting.
• Valid driver’s license with reliable transportation and willingness to travel.
• Eligible to be insured under liability policy with a clean DMV report.
• Excellent communication, interpersonal and time management skills.
• Ability to work independently and in a multidisciplinary team.

Responsibilities

Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.

Who You Are

You are a motivated Speech Therapist who shares our vision of delivering home care. You treat patients in a caring and compassionate manner and help them achieve their goals of staying in their own home whilst receiving treatment.

Travel

This role comprises 100% travel within our service region. You will be traveling to patients’ homes.

Service Region

We have several counties that we service across Northern California specifically Butte County, Shasta County and Yuba Sutter counties and surrounding areas such as Colusa, Glenn and Tehama.

Schedule

This is a Part-time role, with the possibility of expanded hours in the future.

Physical Requirements

Conditional job offer is successfully passing a medical exam. Physical requirement for this role specifically includes

• Sits, stands, bends, lifts, walks, and moves intermittently during working hours.
• Able to lift 50 pounds without assistance.
• Able to drive intermittently daily

Compensation

Pay range is $55.00 to $65.00 per hour. Exact compensation may vary based on skills, experience, and location.
