Horsham Center for Jewish Life in North Wales, PA is seeking a dedicated Full-Time Speech Therapist to join our interdisciplinary care team. You will evaluate residents, design individualized treatment plans, and deliver therapy to support communication, swallowing, cognition, and independence.

The role emphasizes collaboration with nursing, medical staff, dietary, and rehabilitation professionals, along with accurate documentation and family education.

Qualifications

• Current Speech-Language Pathologist license in Pennsylvania required.
• CCC-SLP preferred/required as applicable.
• Master’s degree in Speech-Language Pathology from an accredited program.
• Experience in long-term care, skilled nursing or related healthcare setting preferred.
• Strong clinical assessment, communication and documentation skills.

Responsibilities

• Evaluate residents’ speech, language, communication, cognitive and swallowing needs.
• Develop individualized treatment plans based on assessments and goals.
• Provide interventions to improve communication, cognition and swallowing.
• Monitor progress and modify treatment plans as appropriate.
• Document clinical notes and required reports accurately and timely.
• Educate residents, families and caregivers about plans and strategies.

Horsham Center for Jewish Life is a 324-bed long-term care community committed to providing compassionate, high-quality care and improving the quality of life for our residents.

We are currently seeking a dedicated and compassionate Full-Time Speech Therapist to join our interdisciplinary care team. This is an excellent opportunity for a speech-language professional who is passionate about helping seniors improve communication, swallowing, cognition, and overall independence.

The Speech Therapist will evaluate residents, develop individualized treatment plans, provide therapy services, and work closely with nursing, medical, dietary, and other rehabilitation professionals to support positive resident outcomes.

Key Responsibilities

• Evaluate residents’ speech, language, communication, cognitive, and swallowing needs.

• Develop individualized treatment plans based on assessments and resident goals.

• Provide therapeutic interventions designed to improve communication, cognition, and swallowing function.

• Monitor resident progress and modify treatment plans as appropriate.

• Complete accurate and timely clinical documentation and required reports.

• Educate residents, families, and caregivers regarding treatment plans, recommendations, and strategies.

• Collaborate with nursing, dietary, physicians, and other interdisciplinary team members.

• Participate in care planning and communicate resident progress and recommendations.

• Promote resident safety, independence, dignity, and quality of life.

• Follow applicable facility policies, clinical standards, and regulatory requirements.

Qualifications

• Current Speech-Language Pathologist license in Pennsylvania required.

• Certificate of Clinical Competence (CCC-SLP) preferred/required as applicable.

• Master’s degree in Speech-Language Pathology from an accredited program.

• Experience in long-term care, skilled nursing, rehabilitation, or a related healthcare setting preferred.

• Strong clinical assessment, communication, and documentation skills.

• Compassionate and patient approach when working with seniors.

• Ability to work independently while collaborating effectively with an interdisciplinary team.

• Strong organizational skills and attention to detail.
