Job summary

A healthcare provider is seeking a Speech Language Pathologist (SLP) for a position in a long-term care setting in Logansport, Indiana. Responsibilities include evaluating and treating communication and swallowing disorders, developing effective treatment programs, and educating caregivers. • Identify functional speech defects and establish a plan of care to improve patient's function.
• Implement and revise plans of care as necessary.
• Provide education/instruction to patient, family members/caregivers, and interdisciplinary team.
• Prepare and submit notes per agency policy.
• Perform other duties as required to facilitate delivery of speech language pathology services.

The SLP will be responsible for evaluation and treatment of communication disorders, swallowing disorders, and cognitive deficits, as prescribed by the physician.

• Provides consultation, seeks assistance of others, uses related research and makes recommendations and referrals for the implementation of services.
• Develop and implement effective treatment programs.
• Identify functional speech defects and establishes a plan of care to improve patient's function.
• Implementation and revision of plans of care as necessary.
• Provide education/instruction to patient, family members/caregivers, and interdisciplinary team on recommendations for patient’s care.
• Prepare and submit notes per agency policy.
• Perform other duties as required to facilitate delivery of speech language pathology services.
