Job summary

A healthcare provider is seeking a Speech Language Pathologist (SLP) for a position in a long-term care setting in Logansport, Indiana. Responsibilities include evaluating and treating communication and swallowing disorders, developing effective treatment programs, and educating caregivers. Qualifications

• Must be fully vaccinated for Covid-19, or have medical or religious exemption.

Responsibilities

• Conduct evaluations, assess home environment, and identify equipment needs relative to speech/language function.
• Provide consultation and make recommendations for the implementation of services.
• Develop and implement effective treatment programs.
• Identify functional speech defects and establish a plan of care to improve patient's function.
• Implement and revise plans of care as necessary.
• Provide education/instruction to patient, family members/caregivers, and interdisciplinary team.
• Prepare and submit notes per agency policy.
• Perform other duties as required to facilitate delivery of speech language pathology services.
