Job summary

Qualifications

• Evaluates and treats communication disorders, swallowing disorders, and cognitive deficits as prescribed by the physician.
• Provides patient, family, and team education regarding recommendations for care.
• Prepares and submits notes according to agency policy.

Responsibilities

• Conduct evaluations, assess home environment as appropriate, and identify equipment needs related to speech/language function.
• Develop and implement effective treatment programs.
• Identify functional speech defects and establish a plan of care to improve patient function.
• Implement and revise plans of care as necessary.
• Provide education to patient, family/caregivers, and interdisciplinary team.
• Prepare and submit notes per agency policy.
• Perform other duties as required to deliver speech language pathology services.
