Job summary

A healthcare provider is seeking a Speech Language Pathologist (SLP) for a position in a long-term care setting in Winchester, IN. The SLP will evaluate and treat communication and swallowing disorders, as well as cognitive deficits. Responsibilities include conducting assessments, identifying needs, and educating patients and families. This position can be PRN or part-time, with full-time employees eligible for benefits. Covid-19 vaccination is required unless exempt.

Indiana Fitness Works, Inc. is seeking a Speech Language Pathologist (SLP) for a position in a long‑term care setting in Winchester, IN at Pine Knoll.

The SLP will be responsible for the evaluation and treatment of communication disorders, swallowing disorders, and cognitive deficits, as prescribed by the physician.

Responsibilities

• Conduct evaluations, assess home environment (as appropriate), and identify equipment needs relative to speech/language function.
• Identify functional speech defects and establish a plan of care to improve patient’s function.
• Implement and revise plans of care as necessary.
• Provide education and instruction to patient, family members/caregivers, and interdisciplinary team on recommendations for patient’s care.
• Prepare and submit notes per agency policy.
• Perform other duties as required to facilitate delivery of speech language pathology services.
