Job summary

Indiana Fitness Works, Inc. seeks a Speech Language Pathologist (SLP) for a position in a long term care setting in Logansport, Indiana at Camelot Care Center.

The SLP will evaluate and treat communication disorders, swallowing disorders, and cognitive deficits as prescribed by the physician. Responsibilities include conducting evaluations, developing treatment programs, and collaborating with an interdisciplinary team to support patient care.

Qualifications

• Evaluation of communication disorders and swallowing disorders.
• Development and implementation of treatment programs.
• Education to patient, family and interdisciplinary team.

Responsibilities

• Conduct evaluations, assess home environment, and identify equipment needs for speech/language function.
• Provide consultation and referrals for service implementation.
• Develop and implement effective treatment programs.
• Identify functional speech defects and plan care to improve function.
• Revise plans of care as necessary.
• Educate patient, family and caregivers on recommendations.
• Prepare and submit notes per agency policy.
• Perform other duties to deliver services.
