Job summary

Preferred Home Health Care & Nursing Services is seeking a licensed Speech-Language Pathologist to assess and treat patients with speech and language needs in home health settings.

You will evaluate, develop and implement personalized treatment plans, monitor progress, and collaborate with a care team to optimize patient outcomes while supporting ongoing professional development and quality patient care.

Qualifications

• State licensure or certification required.
• CCC certification required.
• CPR certification required or obtained within 30 days of hire.
• Master’s degree preferred; Bachelor’s degree with relevant experience acceptable.

Responsibilities

• Evaluate, assess, and develop personalized treatment plans for patients with speech and language needs.
• Implement and modify speech therapy interventions to address individual issues.
• Monitor, document, and track patient progress and responses to therapy.
• Celebrate patient successes and adjust approaches as necessary.
