Job summary

Trilogy Health Services, LLC is seeking a Speech Language Pathologist (SLP) to provide a full range of speech-language pathology services, including assessment, treatment planning, and therapeutic interventions, in a team-based environment at Homewood Health Campus in Lebanon, IN.

Responsibilities include evaluating patients, developing therapy plans, delivering therapy to improve communication and swallowing, educating patients and families, and maintaining compliant documentation.

Qualifications

• Master's degree in Speech-Language Pathology and licensure.
• 0-1 years of clinical SLP experience.
• CCC-SLP preferred or required.

Responsibilities

• Evaluates patients to identify speech, language, voice, and swallowing disorders.
• Develops and implements individualized therapy plans based on assessment findings.
• Provides therapy to improve patients’ communication skills and swallowing abilities.
• Educates patients and families about disorders, techniques, and strategies.
• Maintains documentation of evaluations, progress notes, and treatment plans.
• Collaborates with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
