Job summary

Trilogy Health Services seeks a Speech-Language Pathologist to provide comprehensive speech, language and swallowing therapies. You will assess patients, design customized therapy plans, and deliver evidence-based interventions in a collaborative setting, with emphasis on documentation and progress tracking.

Must hold a Master’s degree in Speech-Language Pathology and active state licensure; CCC-SLP preferred.

Qualifications

• Master's degree in Speech-Language Pathology and state licensure current
• Experience with assessment, treatment planning and interventions in an interdisciplinary setting
• Ability to document evaluations, progress notes and treatment plans accurately and timely
• Collaborative work with other healthcare professionals to ensure comprehensive patient care

Responsibilities

• Evaluate patients to identify speech, language, voice and swallowing disorders.
• Develop and implement individualized therapy plans with measurable goals.
• Provide therapy to improve communication and swallowing functions.
• Educate patients and families about disorders and therapy strategies.
• Maintain up-to-date documentation in line with policies and regulatory requirements.
• Collaborate with the interdisciplinary team to coordinate care.
