Job summary

Trilogy Health Services is seeking a Speech-Language Pathologist (SLP) to provide comprehensive language, voice and Swallowing therapies in an interdisciplinary setting. You will assess, plan and implement personalized treatment programs, documenting progress and coordinating care with a team.

Responsibilities include delivering therapy, educating patients and families, and maintaining compliant records. Travel may be required, with a focus on delivering high quality patient care.

Qualifications

• Evaluates patients to identify speech, language, voice, and swallowing disorders.
• Develops individualized therapy plans based on assessments.
• Delivers therapy to improve communication and swallowing abilities.
• Educates patients and families about disorders and therapy strategies.
• Maintains documentation compliant with policies and regulations.
• Collaborates with interdisciplinary team for comprehensive care.

Responsibilities

• Provide a full range of speech-language pathology services including assessment and treatment planning.
• Implement therapy plans with measurable goals for each patient.
• Educate patients and families to support progress in daily life.
• Maintain accurate progress notes and treatment plans.
• Collaborate with other professionals to ensure coordinated care.
• Perform other duties as assigned.
