Sweetwater Care is seeking a qualified Speech-Language Pathologist to provide comprehensive communication and swallowing therapy in a SNF setting. You will evaluate patients, plan therapy, and document progress in collaboration with a multidisciplinary team.

The role involves administering standardized assessments, developing plans of care, and ensuring progress toward goals while adhering to reimbursement guidelines and quality standards.

Qualifications

• Licensed or eligible Speech-Language Pathologist with SNF experience.
• Ability to administer standardized speech-language assessments and swallowing evaluations.
• Experience developing and monitoring treatment plans and progress notes.

Responsibilities

• Direct all aspects of patient care, including evaluation, treatment planning, goals, family education and documentation per guidelines.
• Provide professional treatment for speech, language, hearing, swallowing and cognitive disorders.
• Plan activities and deliver therapy for speech and language disorders.
• Collaborate with other therapists, physicians and healthcare team on patient treatment programs.
• Perform patient evaluations and develop short-term and long-term goals as ordered by physicians.
• Document evaluations, progress notes and discharge summaries in the medical record.
