Job summary

Sweetwater Care is seeking an experienced Speech-Language Pathologist to provide comprehensive evaluation, treatment and documentation for patients with speech, language, swallowing and cognitive disorders in a SNF setting.

You will plan and deliver therapy, collaborate with a multidisciplinary team, and ensure compliance with reimbursement and care standards while advocating for patient needs and outcomes.

Qualifications

• Experience in speech-language pathology in medical settings.
• Knowledge of evaluation and treatment planning for speech, language, swallowing and cognitive disorders.
• Familiarity with Medicare/Medicaid reimbursement systems.

Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to guidelines.
• Provide professional treatment for rehabilitation of speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plan activities and provide therapeutic services for speech and language disorders.
• Advocate for patients; participate in quality assurance and customer service activities.
• Perform patient evaluations and develop treatment plans with short- and long-term goals.
• Carry out patient care plans and determine progression within the care plan.
• Demonstrate knowledge of Medicare, Medicaid and other reimbursement systems.
• Develop and obtain approval for treatment plans from referring physicians.
• Participate in care planning and utilization review meetings.
• Provide patient care appropriate for a SNF setting; administer assessments and swallowing evaluations.
• Perform tests and complex treatments individually or with assistance as needed.
• Collaborate with occupational and physical therapists, physicians and other staff.
• Document evaluations, progress notes and discharge summaries in a timely manner.
• Provide a full range of speech therapy modalities.
• Assist with clinical affiliations of speech therapy assistants.
• Document and prepare Plan of Care and other clinical records ongoingly.
• Ensure delivery of quality rehab services and maintain professional conduct.
• Demonstrate courtesy, compassion and respect to patients and coworkers.
• Maintain compliance with the Compliance Program and Code of Conduct.
• Work with facility on case management needs; perform other duties as assigned.
