Job summary

Sweetwater Care is seeking a Speech-Language Pathologist to evaluate and treat patients with communication, swallowing, and cognitive disorders in a SNF setting. You will develop treatment plans, monitor progress, and document outcomes while coordinating with physicians and therapists.

Responsibilities include administering standardized assessments, conducting swallowing evaluations, and ensuring compliance with Medicare/Medicaid reimbursement guidelines and company policies.

Qualifications

• Ability to administer standardized speech/language assessments

Responsibilities

• Direct all aspects of quality patient care including evaluation, treatment, planning, family education and documentation
• Provide professional speech-language pathology treatment for disorders of speech, language, hearing, swallowing and cognition
• Plan activities and provide therapeutic services for speech and language disorders
• Collaborate with occupational therapists, physical therapists, physicians and other team members on patient treatment programs
• Document patient evaluations, progress notes, and discharge summaries in a timely manner
• Ensure delivery of quality rehab services and adhere to company policies
