Creative Solutions in Healthcare, Inc. seeks a Full-Time Speech Language Pathologist to join our in-house team serving residents through person-centered care and evidence-based practice.

The role focuses on evaluating swallowing and cognitive functions, delivering skilled SLP services, and supporting communication needs across care teams. The ideal candidate holds a Texas license in Speech-Language Pathology, has or is willing to gain SNF/long-term care experience, and stays current with

Qualifications

• Active/Valid Texas license in Speech-Language Pathology.
• In good standing with all regulatory agencies and licensing boards.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Ability to manage patients with different types of personalities.

Responsibilities

• Evaluate residents’ swallowing or cognitive deficits.
• Help residents in goals to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled speech-language therapy assessments and develop treatment plans.
• Develop and implement individualized treatment plans to increase independence.
• Provide AAC systems for severe expressive/language disorders.
• Offer training to clients, families, caregivers, and professionals as needed.
• Coordinate with other professionals for swallowing procedures as ordered by physician.
• Demonstrate competent technical skills per SLP standards of practice.
• Document changes in resident’s condition and maintain records.
• Respond to family requests in a timely manner.
• Coordinate training with Restorative Nursing and care staff.
• Ensure no resident abuse or neglect occurs.
• Observe confidentiality and protect patient information.
• Comply with federal, state, and local professional requirements.
