Job summary

Creative Solutions in Healthcare - East is seeking a dedicated Speech Language Pathologist (SLP) to service our residents with person-centered care in a SNF/long-term care setting. This is a full-time or PRN position supporting cognitive and swallowing needs and implementing individualized treatment plans.

You will educate residents and caregivers, collaborate with the care team, and use AAC systems when appropriate.

Qualifications

• Active Texas license in Speech-Language Pathology.
• In good standing with regulatory agencies.
• SNF/Long term care experience preferred.

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Develop and implement individualized treatment plans.
• Provide AAC systems and communication training.
• Collaborate with other professionals for diagnostic procedures.
• Document billing records and clinical notes.
• Educate residents and caregivers and ensure confidentiality.
