Concierge Home Care in Pensacola, FL is hiring a Home Health Speech-Language Pathologist (SLP). Coverage Area: Pensacola / Escambia County. 1:1 patient care in the home; autonomous scheduling. PRN or 1099 contractor options, mileage reimbursement, and a supportive leadership culture.

Qualifications

• Active, unencumbered Florida Speech-Language Pathologist license.
• Valid driver's license, reliable transportation, and auto insurance.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using streamlined, AI-assisted technology.
