Job summary

Concierge Home Care in Crestview, FL is seeking a Speech-Language Pathologist (SLP) to provide home health services. The role offers flexible scheduling, autonomy, and 1:1 patient care in the home setting.

You will evaluate and treat communication, cognition, and swallowing disorders, develop plans, and collaborate with physicians. Prior OASIS documentation is preferred but not required.

Qualifications

• Active, unencumbered Florida Speech-Language Pathologist license.
• Valid driver’s license, reliable transportation, and auto insurance.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using streamlined, AI-assisted technology.
