Job summary

Concierge Home Care in Pensacola, FL is hiring a Home Health Speech-Language Pathologist (SLP) for flexible PRN work. Earn $75+ per visit with autonomy and the ability to tailor your schedule. This role focuses on 1:1 patient care in the home with minimal drive time.

You will conduct speech, language, cognitive, and swallowing evaluations, develop treatment plans, educate patients and caregivers, and document visits using AI-assisted charting and WellSky/Kinnser EMR.

Qualifications

• Active Florida Speech-Language Pathologist license (or eligibility)
• Valid driver’s license and reliable transportation
• Home health experience preferred but not required
• OASIS/Medicare documentation experience preferred

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations
• Develop individualized treatment plans based on physician orders
• Provide therapy for communication, cognition, and dysphagia
• Educate patients and caregivers to improve functional outcomes
• Collaborate with physicians and interdisciplinary teams
• Complete OASIS documentation as required
• Document visits using AI-assisted technology
