Job summary

Concierge Home Care in Ft. Myers, FL is seeking a Speech-Language Pathologist to deliver 1:1 home health services on a flexible PRN basis. Earn $75+ per visit with autonomy and the ability to tailor your schedule while building meaningful patient relationships.

You’ll evaluate speech, language, cognitive and swallowing needs, develop individualized plans, and document visits using AI-assisted charting. Home-based care minimizes drive time with clustered scheduling and no required weekends.

Qualifications

• Active, unencumbered Florida SLP license.
• Valid driver’s license with reliable transportation.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using streamlined, AI-assisted technology.
