Job summary

Concierge Home Care seeks a Home Health Speech-Language Pathologist (SLP) to deliver 1:1 in-home care across Fort Walton Beach and Okaloosa County. Enjoy a flexible PRN schedule with autonomy and salary potential based on visits, plus Medicare differential pay.

You will conduct evaluations, create physician-ordered treatment plans, and provide therapy for communication, cognition and dysphagia while coordinating with the clinical team and ensuring AI-assisted documentation.

Qualifications

• Active, unencumbered Florida Speech-Language Pathologist license.
• Reliable transportation and valid driver’s license.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using AI-assisted technology.

Your Role as a Home Health Speech-Language Pathologist

Qualifications
