Job summary

Concierge Home Care is seeking a Home Health Speech-Language Pathologist (SLP) for Crestview, FL. This PRN role offers flexible scheduling, pay-per-visit, and the potential to earn $70+ per visit with Medicare differentials.

You will conduct evaluations, develop plans, and provide therapy in patients' homes with AI-assisted documentation tools. Responsibilities include OASIS documentation, collaborating with clinicians, and educating patients and caregivers to improve outcomes.

Qualifications

• Active, unencumbered Florida Speech-Language Pathologist license.
• Valid driver’s license, reliable transportation.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using streamlined, AI-assisted technology.
